Registration for the 2024-2025 SPRING ATHLETICS SEASON is now OPEN!
Registration closes and physicals are due no later than Wednesday, February 12, 2024.

Register for the Athletics Season
You MUST have a cleared physical examination on file dated within 365 days from the start of your season. If you are not sure if you have a physical examination on file that meets the requirements, begin the registration process below. You will be shown the date of your most recent physical examination during the registration process.

1) Sign in to Parent Portal (Trouble signing in? Contact parentaccess@roxbury.org)
2) Select the correct student.
3) Click Student Data in the top menu.
4) Click Forms.

5) In Genesis - Complete the following forms:

- Athletics-Sports Registration
- Athletics-Health Registration
- Athletics-Sports Registration
6) Annual Athletic Physical Exam

- If you DO NOT have a cleared physical examination on file dated within 365 days from the start of your season, you must do the following:

- Print the Annual Athletic Physical Packet (English) or (Spanish)
- Bring the forms to your healthcare provider and have them complete it.
- Upload the completed page 5 (medical eligibility) and Health History Update Questionnaire (HHUQ) in Parent Portal using the Athletics-New Physical Eligibility Document Upload form.
- Print the Annual Athletic Physical Packet (English) or (Spanish)

- If you DO have a physical completed within the date above, continue to the next steps.
- If you DO NOT have a cleared physical examination on file dated within 365 days from the start of your season, you must do the following:
7) Fill out action plan forms (if applicable).

8) Upload all new medical forms

- Click the Athletics-New Physical Eligibility Document Upload form:

The school physician will need to clear your student to participate in Roxbury sports.Once cleared, the eligibility status will be updated in the Genesis Parent Portal.

Late Submission PenaltiesSport physical forms that are submitted after the due date will be subject to the following consequences:
- Within the first school week of due date:
Athlete will NOT be allowed to participate in his/her sport for 1 DAY after he/she has been cleared to participate. - Within the second school week of due date:
Athlete will NOT be allowed to participate in his/her sport for 2 DAYS after he/she has been cleared to participate. - Within the third school week of due date:
Athlete will NOT be allowed to participate in his/her sport for 1 WEEK after he/she has been cleared to participate. - Within four or more school weeks of due date:
Athlete will NOT be allowed to participate in his/her sport for 2 WEEKS after he/she has been cleared to participate.
- In accordance with NJ State Law, the school physician has been contracted by our Board of Education to visit our school 3 times per pre-season.
- The sole purpose for each of these visits is to review and sign off new sports physicals. This does not include already existing valid sports physicals on file in the health office.
- These 3 visits generally fall within the 4 week time frame of the stated due dates. Therefore, if new physical forms are turned in later than the last contractual visit made by our school physician, it will be the athlete's responsibility to incur any additional costs should another visit by the school physician be required to clear those forms submitted after the deadline.
