Medical Statement Request for Special Meals and Milk Substitutions

  • If you would like a “safe menu” (a customized menu to meet your child’s dietary needs and/or restrictions) or other meal accommodation for your child, please complete our "Medical Statement Request for Special Meals and Milk Substitutions form" and submit the supplemental documentation required from your child's physician or licensed medical professional, as explained below.  There are two ways to submit:

  • OPTION 1: 

    Online form, with Supplemental documentation to be submitted via Email or Fax

    Click here to complete the "Medical Statement form" online.  The supplemental documentation required from your child’s physician or licensed medical professional is to be faxed to Maschio's Nutrition Department at (908) 888-2335.  Alternatively, the supplemental documentation may be submitted via email <nutrition@maschiofood.com>. If submitting via email, please use the subject line “SAFE MENU”.

  • OPTION 2: 

    Paper form & Supplemental documentation to be submitted via Email or Fax

    The "Medical Statement form" below, and supplemental documentation required from your child’s physician or licensed medical professional, can be faxed to Maschio's Nutirition Department at (908) 888-2335.  Alternatively, the form and supplemental documentation may be submitted via email <nutrition@maschiofood.com>. If submitting via email, please use the subject line “SAFE MENU”:

  • Please note the "Medical Statement Request for Special Meals and Milk Substitutions form" is required to be completed by the student's licensed medical professional unless updated yearly documentation is on file.  If updated yearly medical documentation is already on file, that documentation is to be provided to Maschio's Nutrition Department via fax at (908) 888-2335, or submitted via email to <nutrition@maschiofood.com>.  If submitting via email, please use the subject line “SAFE MENU”.
